Write the exact dates, "from 12 to 14 December", and the number of days. A clear request is approved faster.
Sick leave
Say how long you have been ill and whether a doctor advised rest. If the school or office asks for a medical certificate, attach it.

How do I write a leave application for school?
Address it to the Principal with the school’s name and city, write "Application for leave" as the subject, give the reason and the dates in two short paragraphs, and end with "Yours obediently", your name, class and roll number.
How do I write a one day leave application?
The same format with one date: "I cannot come to school on 12 December 2026", the reason in one line, and a request for one day’s leave.
Who writes the application for a small child?
A parent. Choose "Sick leave from parents": it is written to the principal about your child, with the child’s class and roll number and your phone number.
Can I write it in Urdu (chutti ki darkhwast)?
Yes. Choose Urdu under Language: the application switches to Urdu wording in Nastaliq, درخواست برائے رخصت.
