A young child’s sick leave application is written by a parent. Write the child’s name, class and roll number, sign as the father or mother, and add a phone number the school can call.
One day or longer?
For one day, the date is enough. For three days or more, say what the doctor advised, and attach the doctor’s note if the school or office asks for it.
Sick leave from the office
Tell your manager the dates, who will handle urgent work, and that you will bring the medical certificate when you rejoin.

How do I write a sick leave application for school?
Address it to the Principal with the school’s name, write "Application for sick leave" as the subject, say you are ill and what the doctor advised, give the dates, and end with "Yours obediently", your name, class and roll number.
How do parents write a sick leave application?
Write it to the Principal about your child: the child’s name, class and roll number, the illness and the dates, then sign as the father or mother with your phone number. The "Sick leave from parents" type fills this in.
How do I write a sick leave application for the office?
Write to your manager or HR with the dates, one line about the illness, who will cover urgent work, and that you will share a medical certificate. End with "Yours sincerely", your name, post and employee ID.
